First Plan Blue offers health care coverage to persons eligible for Prepaid Medical Assistance and MinnesotaCare. First Plan Blue covers families and individuals under these plans in Carlton, Cook, Koochiching, Lake and St. Louis counties.

In addition to medical care available through primary care clinics, First Plan Blue offers a 24-hour, seven days a week nurse line called FirstHelp™. Dental care is available through the Delta Dental network, and mental health and substance abuse counseling services are available through Behavioral Health Services, Inc.

First Plan’s network of providers includes primary care and specialty care clinics that are part of health care systems. Typically, the primary care clinic you choose will determine which specialty care physicians you can access. The health care systems under First Plan are:

CareNorth Health System
CareNorth offers primary care and specialty care clinics. By choosing a CareNorth primary care clinic, you have direct access to CareNorth specialty providers.

SuperiorHealth Medical Group
By choosing SuperiorHealth Medical Group clinics for primary care, you have access to the clinic sites and access to all providers in CareNorth Health System.

Independent Clinics
This system includes only independent primary care clinic providers. Referrals are needed from your primary care clinic to see a specialty provider.

St. Mary’s/Duluth Clinic Health Care System
St. Mary’s/Duluth Clinic Health Care System includes primary care and specialty care clinics throughout Northeastern Minnesota. By choosing this care system you have direct access to all physicians within the St. Mary’s/Duluth Clinic Health Care System.
