FirstPlan Blue Basic

FirstPlan Blue Basic is a voluntary health plan specifically designed for Medicaid members, with or without Medicare. The plan combines health coverage and Medicare part D prescription coverage into one easy program. FirstPlan Blue Basic helps you manage all of your health care needs.

To join, you must:
• Be 18 to 64 years old
• Qualify for Medical Assistance (Medicaid)
• Have both Medicare Parts A and B, or no Medicare
• Live in Carlton, Cook, Koochiching, Lake or St. Louis counties in Minnesota
• Be certified as disabled through the Social Security Administration (SSA) or the State Medical Review Team (SMRT)
• Be a person with Developmental Disability for purposes of the DD waiver, as determined by the Local Agency.

Personalized, one-to-one care
FirstPlan Blue Basic is local. We are based right here in the Northland. Members receive
personal service from professionals who are also neighbors and friends. Another benefit of being with FirstPlan Blue Basic is every member has their own Care Coordinator. This is a person who is familiar with the individual needs of our members. They know that every person has unique issues and concerns. Working with doctors, care providers, family and support people, they make sure everything runs smoothly. This helps members get the help they need. When members or their families need to contact FirstPlan Blue they have one person who already knows them. Care Coordinators will help:
• Arrange doctor and dental visits.
• Coordinate preventative services.
• Manage Medications.

Whatever the level of care a member needs, the coordinator will help them keep their independence and stay healthy. They decide how involved they want the coordinator to be in their care. There is never any charge for this help.
