FirstPlan Blue Minnesota Senior Health Options (MSHO)

FirstPlan Blue MSHO is a voluntary health care program for Seniors age 65+ who are eligible for Medical Assistance (also called Medicaid) and Medicare. FirstPlan Blue MSHO provides the same coverage as Medicare Parts A and B, Part D drug coverage, plus all of the state’s Medical Assistance benefits.

Compassionate care with a personal touch
FirstPlan Blue MSHO combines the benefits of your Prepaid Medical Assistance Program (PMAP) and Medicare. You receive quality coverage, and you don’t need to change doctors. You can use any of our network primary care clinics, dental clinics, behavioral health providers, pharmacies and hospitals. Plus, you’ll get personal attention from FirstPlan Blue’s local customer service staff.

You’ll also have your own Care Coordinator –
a person who’ll know you and understand your needs. Your Care Coordinator will work with your doctors, care providers, and family or support people you choose to make sure you get the help you need. You decide how involved you want your care coordinator to be in your care. Your coordinator can help:

• Arrange doctor, clinic and dental visits
• Coordinate home care
• Assist with hearing aid and eye care
• Manage your medications

Whatever the level of care you need, FirstPlan Blue MSHO will help you stay your healthiest. There is never any charge for your coordinator’s help.

To join FirstPlan Blue MSHO, you must:
• Be at least 65 years old
• Qualify for Medical Assistance
• Have both Medicare Parts A and B, or no Medicare
• Live in Carlton, Cook, Koochiching, Lake or St. Louis counties in Minnesota
