FirstPlan Blue and Delta Dental of Minnesota understand that getting good dental care is very important to you and your family. FirstPlan Blue provides a network of dentists called the Delta Community Dental Care Network. We want to share some important information with you to help you with your dental visits:

• Choose a dentist from among those listed in this directory. Call the dentist’s office to make an appointment.
• If you have questions, call Delta’s Customer Service toll free at 1.800.774.9049. For TTY 1.800.916.9514.
• If you or a family member cannot find a dentist within the network, please follow these steps:

1. Call the Customer Service Department at Delta Dental. They can help you find a dentist. You can reach Customer Service by calling 1.800.774.9049. For TTY/Hearing Impaired call 1.800.916.9514.

2. Once Delta gives you the names of dentists who are in Delta Community Dental Care, call the dental office and ask if they are seeing new FirstPlan Blue patients. If these calls are long-distance and you need help making these calls, tell Delta Dental Customer Service and they will help you.

3. Sometimes a dental office has a waiting list for checkups. If you need care soon, tell the dental office.

4. If you follow these instructions and still cannot find a dentist, call Delta again and Delta will help you find a dentist who is in the Delta Community Dental Care Network or another dentist.

Enrolling in this health plan does not guarantee you can see a particular dentist on this list. If you want to make sure, you should call that provider to ask whether he or she is still part of this health plan. You should also ask if they are accepting new patients. This health plan may not cover all your dental care costs. Read your contract, or “Certificate of Coverage,” carefully to find out what is covered.
